If you click the LAUNCH button on the Launch page and nothing happens, check out your pop-up blocker! Make sure that you are allowing pop-ups from quikformsapp.com in your browser, since all forms are launched in a separate tab. If you allow pop-ups and still nothing happens when clicking Launch, contact Quik! Support.
